Here is my formal request for membership on TOG. I was refered by Cu Daerg. I am a new addictee too Guild Wars living in the Central Valley, Fresno to be specific, CA.

2. You AGREE to abide by the TOG Code of Conduct Guidelines YES.
3. You AGREE to and abide by the TOG Acceptable Useage Policy (AUP) YES.
4. You AGREE to abide by the Forum & Server Guidelines YES.
5. You have read the Frequently Asked Questions (FAQ) Guidelines YES
   
6. You AGREE to abide by the Signature and Avatar Guidelines YES

I Generally play in the evenings (6-10pm PST) and on the weekends.

As you're a Guild Wars player, once the application (which looks perfectly in order) is approved by a Mod, feel free to make your way to the Guild Wars division forum pages, and join up with the guild of your choice.
