I use Minecraft Enhanced 256x because its very similar to the standard one but with high res textures.
That way the colouring etc look the same as the standard texture pack,
largely because I was using some of the others and I thought my construction looked really cool, then switched to standard and it looked rubbish.

This way I enjoy high res textures but without building stuff that looks horrible for people using the standard texture pack

Wow, so many to choose from that look good. I'll have to give a few a try. I am leaning towards the steampunk one for now, but may go with the enhanced for the reasons that the Admiral stated.

Does the patched need to be run just when changing resolutions, or only once?

You run the patcher just once. The catch is that you need to get the new version of the patcher and run it again when Minecraft updates.

I'll try Optifine. I have always heard good things about it for getting a smooth framerate etc.

I had to use Optifine on my Macbook. It was extremely laggy and Optifine was the only recommended one I could find that would actually work on OS X.

I liked to so much with the Macbook, I ended up putting it on my Windows 7 PC. It actually smoothed out the high res much better than what MCPatcher could.
